In-Home Needs Assessment for Tailored In-Home Care

When you find yourself in search of in-home care, you want to make sure you, or your loved one, receives the best one-on-one care possible. Obtaining such services allows you the ability to stay in your own home with the support you need for daily living from a caregiver who is there to exclusively meet your needs. Unlike other facilities, receiving in-home care is a very personal experience. Caregivers are matched with each of their clients based on personality and their tailored care plans. In order to create each individual plan, we first schedule an in-home needs assessment where your personal case manager will assess your needs within your home and get a better understanding of your day-t0-day activities. During this time, your case manager will also be able to:

  • Ask questions about your schedule
  • Note medications you may be taking
  • Assess your ability level
  • Get to know you as well as listen and respond to any concerns
  • Discuss pricing and find what works best for you
  • Answer any questions you may have
