We understand any hospitalization, whether planned or emergent is often stressful and overwhelming for both the patient and their loved ones. Aqua helps our clients navigate through the complexities of Post-Hospitalization, and transitioning from post-hospitaliation or other healthcare facility to home.
After surgery or hospitalization, having the right level of care is critical, that is why our Area Manager will visit you in person to assess your needs and connect you with a care provider who can support recovery and rehabilitation. Continuity of care ensures that the same provider is there to help ease your loved one’s transition from hospital to home. We offer individualized care that promotes healing with compassion and attention to detail. Our team strives to make every recovery journey as comfortable and smooth as possible.
We also offer 1-on-1 Care in Facilities if bedside assistance is needed during a hospital stay, up to 24-hours per day. Referred care provider(s) can continue services in the home after discharge.
Aqua Home Care regularly supports clients in the home following emergency or elective surgeries. Care providers often assist clients recovering from hip and knee procedures, as well as heart, vascular and general surgery. Care may be temporary or long term depending on the condition or recovery plan outlined by your physician or surgeon. Referred care providers can offer physical assistance if mobility is a concern. They can also help around the house, prepare meals, provide transportation to doctor and therapy appointments and much more.
Yes, all care providers must pass a Level 2 Background Screening (requires fingerprinting). Aqua Home Care ensures care providers have met all state requirements prior to being registered and referred to clients. Once registered, they must maintain all credentials in order to remain active with the Nurse Registry.
Yes, we work with all Long Term Care Insurance companies. Aqua Home Care assists clients and family members throughout the entire Long Term Care (LTC) claims process. This includes checking policy benefits, submitting care logs, invoices and other required documentation to help expedite reimbursement for services.
Yes, Aqua Home Care is a Florida licensed Nurse Registry and is bonded and insured. Additionally, referred care providers carry individual professional liability policies and must meet all credentialing requirements set forth by the state.
