Personalized recovery and transitional care programs
We understand any hospitalization, whether planned or emergent is often stressful and overwhelming for both the patient and their loved ones. Aqua helps our clients navigate through the complexities of Post-Hospitalization, and transitioning from post-hospitaliation or other healthcare facility to home. Your dedicated Area Manager will help make appropriate care recommendations and refer care provider(s) qualified to meet your needs and help in your recovery. Care providers can help with personal care, activities of daily living, meal preparation, light cleaning, run errands, and much more. Services can be provided on a short or long-term basis, up to 24 hours daily. Live-in care may also be an option if appropriate.
We also offer 1-on-1 Care in Facilities if bedside assistance is needed during a hospital stay, up to 24-hours per day. Referred care provider(s) can continue services in the home after discharge.
